You need one valid, unexpired government-issued photo ID. It is checked by a third-party identity verification service after this form: you will photograph your ID and take a photo of yourself (a "selfie") to confirm the ID belongs to you.

No guarantee. I understand that enrolling does not guarantee a gift card. Gift cards are issued only if, and to the extent that, Program funds are available or become available, in waiting-list order. I may wait for some time, and I may not receive a gift card at all. One per family. I am enrolling for my family only, and no one else in my family is enrolled. My family may receive no more than one $50 gift card in any calendar month. Identity verification. I authorize the Corporation and its third-party identity verification provider to review my ID, my photo, and the information on this form to confirm my identity and prevent duplicate or fraudulent enrollments. Texts and emails. I agree to receive text messages and emails from the Corporation about my enrollment and gift cards. Message and data rates may apply. I can reply STOP to stop text messages at any time. Gift card use. I will not sell, trade, or transfer any gift card, and I understand that lost or stolen gift cards cannot be replaced. True information. The information I have provided is true, accurate, and complete. I understand that false information, duplicate enrollments, or misuse of gift cards may result in removal from the Program. 12-month enrollment. My enrollment stays active for 12 months from the date it is completed. After that, my family will be automatically removed from the waiting list, and I will need to enroll again if I still need help. Terms and Conditions. I have read and agree to the Grocery Support Program Terms and Conditions. Identity verification (face scan). I consent to the Corporation’s identity verification provider collecting and using a scan of my face and my ID photo to confirm my identity. This data will be used only for identity verification and will be deleted by the provider as soon as the check is complete.

Signature

After you submit this form, we check your ID, then send a one-time code by text message to your mobile phone and a one-time code to your email address. Your enrollment is complete, and your family is placed on the waiting list, only after your ID, phone number, and email address have all been verified.
